General Rates
An Initial Consultation offers an in-depth review of your needs. For quick check-ins or targeted questions, our Brief Consultation provides efficient nurse-led guidance to keep your care plan on the right track.
Initial: $75 per hour | Brief: $40 per 30 mins
Hospital-to-Home Transition Support
Ensure a safe return home with expert oversight. We review discharge orders, verify new prescriptions, and coordinate follow-up care to reduce the risk of readmission and provide your family with total peace of mind.
Starting at $225 per transition
Monthly Advocacy Membership Plans
- Standard Plan: $225/month (up to 3 hours). Enhanced Plan: $300/month (up to 4 hours). Includes the Standard Plan plus more proactive follow-through and structured tracking: Two scheduled family updates per month. Priority scheduling access when available; immediate availability is not guaranteed. 3-month and 6-month Standard and Enhanced plans are available.
Healthcare Navigation & Advocacy
Professional guidance through complex medical systems. We help you understand diagnoses, coordinate with providers, and ensure your loved one's preferences are prioritized during every medical encounter or decision.

Medication List Review & Organization
A comprehensive safety check where we organize complex medication schedules. We help identify potential interactions and prepare specific questions for your physician to ensure safe and accurate adherence at home.
